CATS & DOGS MATTER

CONSENT FORM/WAIVER OF LIABILITY

I, the undersigned, hereby state and acknowledge that:

I have been informed that there are certain medical risks and complications associated with the use of any medication, and the procedure such as but not limited to infection, hemorrhage, allergic reaction, anesthetic drug reaction, anesthesia-induced compromise, and death.

I hereby authorize the use of appropriate anesthesia and pain relief medication needed before or after the procedure. I also understand that the veterinarian performs all surgeries, and utilizes clinic support personnel as deemed necessary.

I understand that Cats & Dogs Matter, partner veterinarians and their staff, and the volunteers will only perform a physical examination, and not a comprehensive cardiac exam, other diagnostic tests, and blood-work prior to the procedure. I also understand that the veterinarians have the right to refuse the procedure on the day should they find my pet(s) unqualified for the procedure.

Spay or neuter surgery which requires general anesthesia, involves the removal of the uterus and ovaries in females, and the removal of testicles in males. In relation to the foregoing, I am aware that my pet(s) will not be checked for pregnancy prior to surgery, and that if pregnant, the fetus/es will be aborted.

If in the course of the treatment, a condition that requires medical attention or additional procedure is discovered, such as hernia repair or the administration of IV fluids, the attending veterinarian may, in his/her absolute discretion, perform such procedure. I consent to these procedures, and agree to pay reasonable additional charges, if any. I also agree to sign a waiver of liability in the event that I decide not to have my pet undergo the needed additional procedure.

Being a responsible pet owner, I undertake to closely monitor important updates during the event, especially in instances when recovery time is longer than usual (beyond the 30-minute average), and conduct personal post-surgery monitoring. I agree that I will also obey the safety precautions as listen in writing, and as explained to me verbally.

I understand that I am financially responsible for any post-operative medical treatment and care relating to this procedure or any other unrelated medical problems of my pet(s).

I will not hold liable Cats & Dogs Matter, its officers, members, partner veterinarians and their staff, technicians, volunteers, and agents, for any problems experienced by the animal as a result of the procedure or the abovementioned risk factors. I further agree to hold harmless any animal shelter, human society or organizations that scheduled the procedure.

I have read and understood all the contents of this form, and authorize Cats & Dogs Matter, partner veterinarians and their staff, and the volunteers of this event to perform spay and neuter surgery, and administer or dispense medications and vaccines as requested in the pet registration form.
